Physical Exams
By UIL and CISD policy, all students must complete and turn in a physical exam every year.
CHS brings in medical staff each May for inexpensive physical exams. Check back in the spring semester for more information.
Any doctor can perform your physical. Most walk-in clinics offer school physical exams for a reasonable cost. Your doctor must use the UIL form:

Upload your completed paperwork to RankOne for the CHS athletic trainers to review and approve. You must complete all 3 steps:
1 – upload your Medical History (checkbox page of the UIL form)
2 – upload your physical exam (doctor page of the UIL form)
3 – complete your signature page and emergency release (online form)

Tips to make sure your paperwork is approved:
- Ensure all signatures and initials are complete
- Explain any “yes” answers on the medical history
- Ensure your scan or photo is clear and includes all 4 corners of the page
